How to Prepare for Your Upcoming Appointment 

We have several different types of appointments that we schedule at our clinic, Telehealth, In-Office Appointments, and Cystoscopy with Consultation. In preparation for your upcoming appointment, please see the information below to familiarize yourself with what can be expected. We also request that you complete the Intake Form and other requested documents/labs (if applicable), and review the policies.

If you are waiting for an initial appointment, please review our policies. Please contact you referring doctor if you are not an active patient at our clinic and have questions or concerns regarding your referral,

The Medical Services Plan (MSP) requires referral from another physician for specialist urology consultation. We cannot accept self-referrals, please see your primary care/family physician.
